It is common for students to complain of bumps, blows and jolts to their head or body after physical activity (e.g. PE or recess). As a matter of high precaution, any head injury is reported, parents are notified and the student is sent to the health room for observation of a possible concussion. Concussions signs and symptoms can present soon after the injury or may appear days or even weeks later. Thus, in addition to a phone notification to the parent, a report of the observed signs or symptoms (if any) and a fact sheet of signs and symptoms will be sent home with the student to help parents with ongoing monitoring.
Child Medications: Oral Medications & Forms
If your child requires oral medication (both prescription and non-prescription) during school hours, please remit a Medical Authorization Form to the school office. The forms are available at the office; we will accept any standard form from any provider’s office.
Forms must be SIGNED and COMPLETED by a licensed provider (MD, DO, PA, Nurse Practitioner or Dentist). All medications must be in their original, labeled container.
For the safety of your child, all medications must be brought to the school office by an adult and will be dispensed and recorded in the office. No medication is allowed to be carried to school by a student, except by students authorized to carry rescue inhalers and/or Epi-pens.
Immunization Records Review
Washington law requires that all students attending schools must provide proof of immunization or present medical exemption or a personal/religious exemption signed by a licensed medical provider on or before entering school. Immunizations records are reviewed and updated at the beginning of every school year. You can check the status of your child’s immunization record by visit www.myirmobile.com or contacting your child’s primary health provider. We are currently in the process of reviewing all registered students' records and will be reaching out to individual families with incomplete records.
