Saint Charles is pleased to host another vaccine clinic in Fall of 2024. Please bring with you a copy of your insurance card and the below forms. If you want us to escort your child to their appointment, please have them bring their forms to school with them on the day of the clinic, November 4th.
All adults need the adult form and the demographic form. All students under 18, need the pediatric form and the demographic form.
